Provide direct daily support and life skills training to individuals with disabilities.
Assist with personal care, household tasks, and safe community participation.
Implement person-centered service plans and support individual goals.
Document services, progress, and incidents accurately and on time.
Monitor health, behavior, and safety, reporting concerns promptly.
Collaborate with families, caregivers, and clinical team members.
Transport or accompany individuals to appointments and activities.
Uphold dignity, respect, and inclusion in every interaction.
