Description
As a Kidney Care Options Educator at U.S. Renal Care, you will guide patients and families through the complexities of chronic kidney disease, dialysis, and treatment options. You will provide one-on-one and group education, helping patients understand their diagnosis, compare therapies, and make informed choices that align with their goals and lifestyles. Working closely with nephrologists, nurses, and social workers, you will support coordinated care and smooth transitions between modalities. In our collaborative culture focused on integrity, excellence, and empowerment, you’ll contribute directly to patient outcomes while growing professionally through ongoing learning and teamwork.Provide individualized education to patients and families on CKD, dialysis, transplant, and conservative care options.
Explain dialysis modalities and help patients make informed treatment decisions.
Collaborate with nephrologists, nurses, and social workers to support care plans and smooth transitions of care.
Develop and deliver group education sessions and educational materials tailored to diverse literacy levels.
Document assessments, education, and follow-up in the EHR in a timely, accurate manner.
Track patient engagement and outcomes, adjusting education strategies as needed.
Advocate for patient needs and empower patients to participate actively in their care.
Stay current on kidney care guidelines, therapies, and best practices to ensure accurate education.
Support referrals to appropriate services, including nutrition, social work, and transplant centers.
Promote a culture of integrity, teamwork, and excellence aligned with U.S. Renal Care values.
