Description
ChenMed seeks a Patient Educator to empower seniors with complex chronic conditions through high-touch, value-based primary care. In this team-oriented role, you’ll deliver individualized and group education, promote self-management, and support care plan adherence to reduce hospitalizations and improve quality of life. Working closely with physicians, nurses, and care teams, you’ll assess health literacy, address barriers, and engage caregivers. Ideal candidates bring strong communication skills, experience with seniors, and a passion for mission-driven, patient-centered care in underserved communities.Deliver individualized and group education for seniors with complex chronic conditions to improve self-management and adherence.
Collaborate with physicians, nurses, and care teams to design and reinforce patient-centered care plans in a value-based model.
Assess health literacy, barriers to care, and social determinants; tailor education materials and approaches accordingly.
Track and document patient interactions, outcomes, and progress in the EHR to support coordinated care and quality metrics.
Engage families and caregivers, providing tools and resources that support safe transitions of care and reduced hospitalizations.
Participate in quality improvement initiatives, community outreach, and multidisciplinary huddles to enhance patient outcomes.
Patient education and counseling
Chronic disease management (e.g., diabetes, CHF, COPD)
Care coordination and navigation
Health literacy assessment and communication
Motivational interviewing
Electronic Health Record (EHR) documentation
Care plan development and follow-up
Population health and value-based care principles
Geriatric care knowledge
Data tracking and outcome reporting

Certified Health Education Specialist (CHES) or Master Certified Health Education Specialist (MCHES) preferred
Registered Nurse (RN), LPN/LVN, or other clinical licensure a plus
Certification in chronic disease management or diabetes education (e.g., CDCES) preferred
Bilingual skills (e.g., English/Spanish) beneficial for senior and underserved populations
